Discriminiation Report Form

Before you begin, if you have an immediate safety concern, please contact Campus Safety (502-873-4444) or call 911.

Students, faculty, or staff who have experienced or witnessed discrimination may use this online form to make the university aware of the incident. Reports of discrimination or harassment are directed to the Dean of Students.

The university's policies that prohibit discrimination offer an effective and efficient process for individuals and/or groups to report allegations of discrimination.

Individuals experiencing the reported discrimination, harassment, or sexual misconduct will be contacted by the Dean of Students or Title IX Coordinator.
